I always wash my T shirts first, using very little detergent and add 1/4 Cup of clear vinegar, on the perma-press cycle, then dry them on the perma-press cycle. Take them out of the dryer immediately, to prevent wrinkles, and press them lightly with a steam iron. Lay them on a flat, firm surface, not the ironing board to assure you bond the transfer firmly to the fabric, following the instructions on the transfer from here. When washing the shirts later, always turn them inside out and use the gentle cycle, cold water and dry them in a cool dryer. This will extend the life of your images. I hope this helps. I know how frustrating this is...it happened to me, many times, before I learned a few tricks.

Turn item inside out and if you can use cold water wash and rinse. Hang dryer is best, but if you have to dry in dryer set dryer on delicate.

The number one reason the transfer cracks off is your fabric softener. The second is the heat in the dryer is too high. Try eliminating the fabric softer, then only dry to "damp" then hang to dry.
But before you ever wash the item, try setting the transfer by pressing it a second time before peeling off the transfer paper (taking care to not let the transfer cool too much... if the image is lifting, re heat

Are you drying them in the dryer? It might help if you hang them to dry instead of putting them in the dryer. Also, did you prewash the shirts before putting the transfers on? If the shirts are shrinking, that can certainly cause the transfers to flake and peel off. You are doing the right thing in turning the shirts inside out and using cold water.
